An epidemiological investigation traced the source of the outbreak back to sprouts that had been distributed to wholesalers, grocery stores, restaurants, and other retail outlets in Oregon, Washington, Alaska and California. Consumers and retailers were instructed to discard any produce they still had.
Harmony Farms instituted a number of changes in its sprout growing procedures, following an FDA inspection of its facilities, in order to prevent a reoccurrence; however, the Oregon Department of Human Services issued another News Release on November 26, 2003, advising of another Harmony Farms alfalfa sprout recall. Oregon health officials had again identified Harmony Farms sprouts as the probable cause of an outbreak of Salmonellosis. Six Oregon residents and one Washington case were linked to this new Salmonella outbreak.
